Frecuencia de sepsis neonatal en la readmisión hospitalaria en recién nacidos enviados a alojamiento conjunto

Resumen

Introducción: la readmisión hospitalaria en los recién nacidos suele ser frecuente, general- mente asociada a la edad materna, variación geográfica, situación socioeconómica, partos de pretérmino, nacimientos por parto cesárea y el alta temprana, que en la actualidad es considerada una causa o factor determinante para la readmisión. A su vez, puede contribuir con la presencia de sepsis neonatal, entidad que compromete la vida del neonato y provoca un trastorno en el desenvolvimiento del círculo familiar.

Material y métodos: se realizó este estudio descriptivo, observacional, exploratorio y de corte transversal, no experimental, con la finalidad de determinar la frecuencia de sepsis neonatal en la readmisión hospitalaria recién nacidos previamente enviados a alojamiento conjunto y que antes de los 28 días de vida extrauterina fueron traídos al servicio de emergencias del Hospital Materno Infantil San Lorenzo de Los Mina, en el período enero-marzo 2016.

Resultados: hubo un total de 480 ingresos en la unidad de neonatología, con diagnóstico de sepsis neonatal 31,2 %; de estos, un 27 % fue ingresado en sala externa de neonatología, corres- pondiendo así a todos los recién nacidos que al nacimiento fueron enviados a alojamiento conjunto y egresados aparentemente sanos. La frecuencia de ingresos por sepsis neonatal en la readmisión hospitalaria fue un 27 % de todos los casos de sepsis, 85,4 % de los ingresos totales a sala externa de neonatología. Madres adolescentes en 10, 6 %. La vaginitis estuvo en 97,4 % e infección de vías urinarias 69,2 %; y fueron los antecedentes personales patológicos más frecuentes. En el 87,18 % fueron a término. Sexo masculino en el 64,10 %. Con nacimiento intrainstitucional en 71,79 %.

Conclusiones: es de vital importancia contar con promoción y prevención en salud, desde antes del embarazo, el cual debe tener un control riguroso, así como tomar en cuenta los antecedentes manteros antes del alta de los neonatos.

Abstract

Introduction: Hospital readmission in newborns is usually frequent, generally associated with maternal age, geographical variation, socioeconomic status, preterm births, births by cesarean section and early discharge, which is currently considered a cause or determining factor. For readmission. In turn, it can contribute to the presence of neonatal sepsis, an entity that compromises the life of the newborn and causes a disorder in the development of the family circle.

Material and methods: this descriptive, observational, exploratory and cross-sectional, non-experimental study was carried out with the purpose of determining the frequency of neonatal sepsis in hospital readmission of newborns previously sent to rooming-in and who, before 28 days of extrauterine life were brought to the emergency service of the San Lorenzo De Los Mina Maternal and Child Hospital, in the period January-March 2016.

Results: There were a total of 480 admissions to the neonatal unit, with a diagnosis of neonatal sepsis 31.2%; Of these, 27% were admitted to the external neonatal ward, thus corresponding to all the newborns who were sent to rooming-in at birth and apparently healthy discharged. The frequency of admissions for neonatal sepsis in hospital readmission was 27% of all cases of sepsis, 85.4% of total admissions to the neonatal outpatient ward. Teenage mothers in 10, 6%. Vaginitis was in 97.4% and urinary tract infection 69.2%; and they were the most frequent pathological personal antecedents. In 87.18% they were at term. Male sex in 64.10%. With intra-institutional birth in 71.79%.

Conclusions: It is vitally important to have health promotion and prevention, from before the pregnancy, in which it must have a rigorous control, as well as taking into account the antecedents before the newborns are discharged.
