The vestigial tail, also known as the human true tail, pseudo tail, or lumbosacral appendix, is a rare condition associated with abnormal formation of the neural tube and hidden dysraphism. We report the case of a full-term male newborn of Dominican nationality, delivered vaginally, who was admitted to the neonatal unit. The newborn presented a vestigial tail located along the midline in the sacral region. The mother had regular prenatal care with no notable complications. Upon identification of the lumbosacral appendix during the physical examination, a multidisciplinary approach was initiated involving neonatology and pediatric surgery to rule out a neural tube defect and confirm the diagnosis.
